29 CFR 1926.501(b) (13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented:                                                                                                             This violation occurred on October 02, 2012, at the jobsite located at 1534 Abrams Road in Dallas, Texas 75214, where at least four (4) employees performing roofing work were not protected from falling to the lower level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the   ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder   was used to gain access:     This violation occurred on October 02, 2012, at a jobsite located at 1534 Abrams Road in Dallas, Texas 75214, where the side-rails of the ladder that at least one (1) employee used to access/egress the roof did not extend at least 3-feet above the upper landing surface.
