29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) Jobsite, 2525 5th Avenue, McKeesport, PA:  On or about 10/04/12 an employee was positioned on the top of the parapet wall directing the crane operator who was placing a load on the roof and no fall protection was in place to prevent the employee from falling approximately 20 feet to the ground below.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:    (a) Jobsite, 2525 5th Avenue, McKeesport, PA:  On or about 10/04/12 retraining had not been conducted when inadequacies in an affected employee's knowledge of and use of fall protection systems indicated the employee had not retained the requisite understanding and skill necessary to protect themselves from exposures to fall hazards.  An employee was exposed to a fall hazard of approximately 20 feet and no fall protection system was in use.  The employee had received training on fall protection and personal fall arrest equipment was on site and available for use.
