29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about 10/04/2012, at the job site - employees conducting roofing work were permitted to work from the roof without any type of fall protection, exposing them to fall hazards of 24 feet 6 inches, 18 feet 8 inches and 13 feet 8 inches to the ground below. MILBAR ROOFING, INC. WAS PREVIOUSLY CITED FOR VIOLATIONS OF THE OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(11) WHICH WAS CONTAINED IN OSHA INSPECTION 315498048, CITATION 01 ITEM NUMBER 01A, ISSUED ON 04/08/2011, AND BECAME A FINAL ORDER ON 05/03/2011, WITH RESPECT TO A WORKPLACE LOCATED ON 22319 CORTEZ BLVD. BROOKSVILLE, FL 33601.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. On or about 10/04/2012, at the job site - the ladder used for access to an upper landing surface only extended one rung above the landing surface exposing the employees to a 13'9" fall hazard.
