29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: a) Located in the double disc department the pit areas located on the right, left, and rear sides of the Blanchard number 59 grinding machine did not have protective floor grating or equivalent cover protection to prevent employees from inadvertently stepping and or walking off into the twenty-one inch deep by twenty-four inch wide and greater open sided floor.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) There was no program on energy control procedures, employee training, and periodic inspection, to include providing locks and training to employee who performed maintenance and changing spindle liner on Dae-wood CNC lathe machines, and who performed repair on the Beeley Feed Machines, and the Doulble Disc machines.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) In the Grinding department the Blanchard number 71 grinding machines did not have adequate guarding in place to fully protect the operators, in that the sliding door closure did not prevent the operator from inadvertent contact and or from reaching over the guard into grinding turret.

29 CFR 1910.219(e)(3)(i): Vertical or inclined belt(s) were not enclosed by guard(s) conforming to the requirements specified at 29 CFR 1910.219(m) and (o). a) The employees who operated Double Disc number 35 and 64 were exposed to caught-in hazard(s) in that the 2,350 RPM electric drive motors and the belt pulley systems that powered the machine part conveyor was not covered and guarded to prevent the operator from inadvertent contact during part loading operations and scrap ben removal. b) The employees who cleaned fluids that leaked from behind the Grand Rapid surface grinder onto the floor were exposed to caught-in hazard(s) in that the belt and pulley system that was powered by off the electric motor to the hydraulic pump was not covered and guarded to prevent the operator from inadvertently contact during their cleaning operations.
