29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) Keystone Cap Company, 927 Blunston Street, Columbia, Pennsylvania - Employees performed servicing and maintenance activities, such as die-changing, on a variety of machinery and equipment, including, but not limited to, the Bliss 19 and 19C OBI full revolution mechanical power presses, and the employer did not develop and implement an energy control program, consisting of machine specific energy control procedures, group lockout/tagout, periodic inspections, employee training, etc., on or about October 4, 2012.

Each press operator maintained continuous pressure on the foot pedal, which allowed the press to cycle continuously, while the operator hand-fed metal strips into the point of operation, on or about October 4, 2012.  (b) First Floor, Liner Department - The Bliss No. 19 OBI full revolution mechanical power press, used to punch liners (a paper material), did not incorporate a single-stroke mechanism.  The press operator maintained continuous pressure on the foot pedal, which allowed the press to cycle continuously, while the operator hand-fed strips of liner material into the point of operation, on or about October 4, 2012.

29 CFR 1910.217(c)(2)(i)(a):  Point of operation guards on mechanical power presses did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guards:  (a) First Floor, Press Area - The fixed barrier guards on the three Bliss No. 19C OBI full revolution mechanical power presses, used to punch metal, had openings measured at 2 inches around the bottoms of the guards and openings measured at 2.75 inches to 5 inches around the tops of the guards, which permitted entry of the hands or fingers into the points of operation by reaching over and under the guards, on or about October 4, 2012.  (b) First Floor, Liner Department - The fixed barrier guard on the Bliss No. 19 OBI full revolution mechanical power press, used to punch liners (a paper material), had openings measured at 1 inch around the bottom of the guard and openings measured at 1.5 inches around the top of the guard, which permitted entry of the hands or fingers into the points of operation by reaching over and under the guards, on or about October 4, 2012.

29 CFR 1910.217(b)(4)(i):  Pedal mechanisms on mechanical power presses with full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals:  (a) First Floor, Press Area - Three Bliss No. 19C OBI full revolution mechanical power presses, used to punch metal, did not have adequate foot pedal protection.  The employer had installed under the foot pedal of each of these presses a mechanism consisting of a rod supported by two blocks of wood.  This mechanism prevented activation of the foot pedal as long as the rod component remained pushed into the block of wood on the right side on the mechanism.  However, once the operator moved the rod out of the block of wood on the right side of the mechanism, to permit the activation of the foot pedal, the foot pedal no longer had protection from unintended operation from falling or moving objects or by accidental stepping onto the pedal because the rod did not automatically reinsert itself into the block of wood on the right of the mechanism as soon as the operator stopped pressing the foot pedal, on or about October 4, 2012.  (b) First Floor, Liner Department - The Bliss No. 19 OBI full revolution mechanical power press, used to punch liners (a paper material), did not have adequate foot pedal protection.  The employer had installed under the foot pedal of this press a mechanism consisting of a rod supported by two blocks of wood.
