OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to crushing hazards and or scaffold  collapse.     (B) Location: Cornerstone Square, Westford, MA:    On or about 10/3/12, employees were exposed to crushing hazards while the mast climber scaffold was not installed correctly. Inadequate spring pins were used and scaffold support would not withstand all load forces imposed by the machine.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access.    (a) Location - Corerstone Square, Westford, MA:  On or about 10/3/12, employees were exposed to the hazard of falling while accessing scaffold platform using cross-bracing.

Or, on roofs 50-feet (15.25 m) or less in width.
