At the workplace, the employer did not ensure that all stationary equipment was properly wired. A pallet shrink wrap machine located near the bay doors on the loading dock, had an extension cord wiring into the control box and plugged into a near-by receptacle outlet, exposing employees to a fire hazard.

At the workplace, the employer used an extension cord to charge a motorized pallet jack near the office. The extension cord was ran from a 110v electrical receptable inside the office, through a door, to the motorized pallet jack outside the office, exposing employees to a shock hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a. At the workplace, the employer did not provide extensions cords that were free from defects, exposing employees to a shock hazard. Employees used an extension cord to charge a motorized pallet jack. The end of the extension cord had the strain relief pulled out of the plug.
