29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    WORKSHOP: The mployeer had not established and implemented a site specific hazardous energy control lockout/tagout program consisting of machine specific step-by-step written procedures and employee training for the employees during servicing and maintenance of equipment, such as (but not limited to) Radio frequency machines.

WORKSHOP:  The employees, as authorized, affected and/or other types were not provided with training and information in the controlling of potentially hazardous and unexpected releases of revised energy sources (lockout/tagout) during servicing and maintenance of equipment, such as (but not limited to) the Radio Frequency machines.
