The attachment had not been approved by the manufaturer and the plates had not been updated to reflect capacity, on or about 10/25/2012.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a) At the site - a Yale Forklift Model # GNUAE084 was equipped with an "in house" manufactured front end attachment measuring 6 foot long by 2 foot high and 63 inches wide used for lifting foam materials. The forklift had not been marked to show the changes in capaciity with the attachment, on or about 10/25/2012.
