29 CFR 1926.403(b)(1):     Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations:    (a).  Worksite located at 3730 Mitchell Ave., St. Joseph, Missouri- Employees were exposed to shock or electrocution hazards in that cord powering the radial saw had been severed and then wired and taped back together.

29 CFR 1926.100(a):     Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    (a).  Worksite located at 3730 Mitchell Ave., St. Joseph, Missouri- Employees were exposed to overhead hazards and were not protected from impact by the use of hard hats.

29 CFR 1926.451(a)(6):     Scaffolds shall be designed by a qualified person and shall be constructed and loaded in accordance with that design. Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section.    (a). Worksite located at 3730 Mitchell Ave., St. Joseph, Missouri- Employees were exposed to a fall hazard in that the platform being used by the employees had not been designed by a competent person in the structural design of the platform in which capacities had not been tested for all the vertical as well as horizontal stresses that are applied to the platform which was being hoisted by a forklift.     (b). Worksite located at 3730 Mitchell Ave., St. Joseph, Missouri- Forkifts were being used to elevate employees. The manufacture's instruction prohibit the use of these forklifts to raise employeees.

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold.     (a).  Worksite located at 3730 Mitchell Ave., St. joseph, Missouri- Employees were exposed to a fall hazard in that the employees were lifted in a platform that did not have adequate guardrailing.     (b).  Worksite located at 3730 Mitchell Ave., St. Joseph, Missouri- Employees were exposed to a fall hazard in that employees were working on top of sheets of plywood supported by a forklift.
