29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Firestone Complete Auto Care - On or about October 3, 2012, employees used aluminum extension ladders to gain access to the roof without the side rails of the ladder being extended at least 3 feet above the upper landing surface of the roof nor was the ladder secured at its top to a rigid support, exposing employees to fall hazards of up to approximately 16.5 feet onto concrete ground. ABATEMENT CERTIFICATION IS NOT REQUIRED
