29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Jobsite, 5501 Elgin Street, Pittsburgh, PA: On or about 10/04/12 an employee was sitting on a box gutter approximately 30 inches wide with an unprotected side approximately 30 feet above the ground and no fall protection system was in place to prevent the employee from falling.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite, 5501 Elgin Street, Pittsburgh, PA: On or about 10/04/12 the employer did not provide a training program for employees whose work exposed them to fall hazards to ensure the employees were able to recognize fall hazards and what procedures and methods could be used to minimize such hazards. An employee was exposed to fall hazards of approximately 30 feet and no training had been provided to the employee on the procedures to follow to protect themselves from exposure to falls.
