On or about October 3, 2012, in the dismantling department employees used custom made fork attachments on the Hyundai (model HL757TM-9 S/N HIHL02TB00000037) front end loader to lift, transport, and suspend loads on the tips of the custom made fork attachments.  While this unsafely arranged load (load supported by the tips of the forks) was suspended an additional employee placed their body underneath that load.  Additionally, the custom fork attachment was not inspected, tested, or marked with the safe maximum working capacity.
