OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees    a.  At the workplace, employees were operating a high lift order picker truck with a self retracting lifeline and a body belt to protect against a fall hazard.  The body belt was not permitted to be used with a fall arrest system such as a self retracting lifeline due to the lack of fall arrest force distribution exposing employees to internal injury.

29 CFR 1910.305(b)(1)(ii):     Unused openings in cabinets, boxes, and fittings shall be effectively closed.    a.  At the workplace, in the coil room, a junction box attached to the ventilation system had two approximately one inch openings exposing open wiring that were not effectively closed with a knockout plug.    b.  At the workplace, in the coil room, a receptacle box attached to the ventilation system junction box had an approximately one inch opening exposing open wiring that was not effectively closed with a knockout plug.

29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    a.  At the workplace, in aisle CO1A, there was an electrical outlet box mounted on a structural steel beam that did not have a  faceplate installed to prevent employee exposure to contact with electrical current from exposed open wiring.

29 CFR 1910.132(d)(2):     The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    a.  At Palmer Donavin, located in Columbus, Ohio had employees that would use personal protective equipment to minimize hazards associated with their job tasks, but the employer had not performed a written hazardous assessment to certify which PPE should be worn while performing different tasks.

At the workplace, at the East end of the main storage aisle, a portable fire extinguisher was not mounted.    b.  At the workplace, at the North East exit door of the building, a portable fire extinguisher was not mounted.    c.  At the workplace, at the North East battery charging station, a portable fire extinguisher was not mounted.
