29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel: Press Room: The bench grinder was not equipped with a tongue guard on the right side with the abrasive wheel.

29 CFR 1910.219(m)(1)(i): Standard conditions were not secured by the use of the following materials: expanded metal, perforated or solid sheet metal, wire mesh on a frame of angle iron, or iron pipe securely fastened to floor or to frame of machine: Baum folder: The cover for the motor was removed and replaced with a piece of cardboard that was taped to the front of the machine. Additionally, a hole was cut into the cardboard exposing the drive belt for the motor.
