29 CFR 1910.147(c)(4)(ii):     Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):    On or about October 3, 2012:    Manufacturing building - Employees performed maintenance and repairs on the roaster machine, coffee, tea lines, and fleet maintenance.  The employer did not ensure energy control procedures included step-by-step instructions for locking and/or tagging out equipment.

29 CFR 1910.134(c)(2)(ii):     The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    On or about October 3, 2012:    Manufacturing Building - An employee responsible for removing residue and dust accumulation from coffee was provided half-face respirators for voluntary use.  The employer did not ensure the employee was medically evaluated and trained on the properly cleaned and stored.    Excelso Coffee & Tea Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(c)(2)(ii), which was contained in OSHA inspection number 314209545, citation number 2, item number 3 and was affirmed as a final order on 08/22/11, with respect to a workplace located at 6700 Dawson Blvd, Norcross, GA 30093.

29 CFR 1910.303(g)(1)(ii):     The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:    On or about October 3, 2012:    a)  Manufacturing Building - Material was stored in front of the electrical disconnects along the back wall, behind the coffee lines.  The employer did not ensure the working space was free of stored materials.    b)  Manufacturing Building - Material was stored in front of the electrical panel located on the side wall, next to coffee line #4.  The employer did not ensure the working space was free of stored materials.    Excelso Coffee & Tea Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(g)(1)(ii), which was contained in OSHA inspection number 314209545, citation number 2, item number 5 and was affirmed as a final order on 08/22/11, with respect to a workplace located at 6700 Dawson Blvd, Norcross, GA 30093.
