29 CFR 1904.39(a): The employer did not notify the Area Office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor within eight (8) hours after the death of any employee. On or about August 14, 2012 an employee was electrocuted while removing copper wire from an electric transmission pole, and the employer did not report the fatality to the Occupational Safety and Health Administration within eight hours. OSHA was notified on September 25, 2012.
