29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Jobsite - On or about October 03, 2012 an employee was operating a Genie model S-60X aerial lift without a safety harness and lanyard exposing the employee to a fall height of 14 feet.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes by covers. Jobsite - On or about October 03, 2012 employees were exposed to unprotected open holes while erecting wood framed walls. There were 11- 24 inch square openings, structural steel footings, that were 11 inches in depth.
