29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) Jobsite - on or about October 2, 2012 - the TCM forklift, model FG15, serial# 259H, was not inspected by a competent person for defect and damage. Observed conditions include; broken right side mirror, broken left headlight and the LPG cylinder was not properly secured to the machine.
