OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees Employees were exposed to burns while working on line 4A the Davis-Standard Thermatic Extruder that had the rupture tube capped not allowing for relief of excessive pressure. Feasible means of abatement could have been achieved by following the Davis-Standard manual for Thermatic, Mark V & Vl and Fibermaster owners manual. This document was entitled, Davis-Standard manual for Thermatic, Mark V & Vl and Fibermaster, and it recommended the following: Rupture disc must be installed before operating extruder.

If such hazards are present, or likely to be present, the employer shall: Employees are exposed to burns while working on line D4A, Davis-Standard Thermatic Extruder. The employer failed to conduct a hazard assessment when the rupture disc was replaced with a solid cap.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. Instance 1: Employees are exposed to fire hazards, the cover plate was missing on junction boxes on the southwest side of the upper platform next to line D4A. Instance 2: Employees are exposed to fire hazards, the cover plate was missing on the junction box between the hopper feed and the D4A extruder.

29 CFR 1910.305(g)(1)(iii): If used as permitted in paragraphs (g)(1)(ii)(C), (g)(1)(ii)(G), or (g)(1)(ii)(I) of this section, the flexible cord shall be equipped with an attachment plug and shall be energized from an approved receptacle outlet. Employees working around the D4A extruder are exposed to electrical shock. Power cords being plugged into the die had damage to the flexible conduit exposing electrical wiring.
