Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about October 5, 2012, during roofing activities including but not limited to installing a standing seam metal roof, the employer did not ensure all employees on the roof were using a fall protection system/device on a low sloped roof with a fall height approximately 10.8 feet in the front and 13.7 feet in the rear.
