29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Establishment: The employer's lockout/tagout program was not fully developed and implemented by the employer, where employees maintain/service machinery and equipment, including but not limited to the Bruderer (60 Ton)/BSTA 60 Stamping Press, Stamping Press, and other machinery in the establishment, as periodic inspections were not conducted and information in the employer's program was not site specific. Violation occurred on or about 10/10/2012.
