29 CFR 1915.56(a)(2): Current carrying parts passing through the portion of the holder which the arc welder or cutter grips in his hand, and the outer surfaces of the jaws of the holder were not fully insulated against the maximum voltage encoutered to ground. (a) Welding shop, stinger on welding machine had electrical tape covering a screw that was not properly recessed on the stinger handle. Tape was ineffective it had a hole over the screw. Screw is fastened to the main body of the stinger and when on would be energized causing employee to be exposed to an electric shock.
