29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a) House roof, intersection of Ohio and Willard Streets, White Oak, Pennsylvania, 15131: Observed on or about October 10, 2012, employees engaged in roofing activities on a two story residential dwelling were not protected from falling by guardrail systems, safety net systems, or personal fall protection systems. The slope of the roof was approximately 8 in 12 (vertical to horizontal). The employees were exposed to a fall hazard of approximately 20 feet to ground below. b) Garage roof, intersection of Ohio and Willard Streets, White Oak, Pennsylvania, 15131: Observed on or about October 10, 2012, employees engaged in roofing activities on a one story garage were not protected from falling by guardrail systems, safety net systems, or personal fall protection systems. The slope of the roof was approximately 2 in 12 (vertical to horizontal). The employees were exposed to a fall hazard of approximately 8 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: a) Residential house, corner of Ohio and Willard Streets, White Oak, Pennsylvania, 15131: Observed on or about October 10, 2012, employees engaged in roofing activities on a two story house were not trained in the recognition of fall hazards and the procedures to be used to reduce or eliminate these hazards. The employees were exposed to a fall hazard of approximately 20 feet to ground below while working on the roof of the house. The slope of the roof was approximately 8 in 12 (vertical to horizontal).
