29 CFR 1926.451(b)(5)(ii):     The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:    Location:  At or near 11576-11598 Westpark Place Dr., Saint Louis, MO  63146.    The employer did not ensure that platforms did not extend beyond the end brace more than 18".  The platform of the 2 pump jack scaffold extended beyond the supports as follows:    a.)  The north end of the west scaffold extended approximately 2'5".  b.)  The south end of the west scaffold extended approximately 5'7".  c.)  The north end of the east scaffold extended approximately 9'5".  b.)  The south end of the east scaffold extended approximately 7'5".

29 CFR 1926.451(b)(7):     On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:    Location:  At or near 11576-11598 Westpark Place Dr., Saint Louis, MO  63146.    The employer did not ensure that platforms overlapped over braces or supports on the 2 pump jack scaffolds.   The pump jack scaffolds had platforms overlapped without any support and were secured with ratcheting straps.

29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    Location:  At or near 11576-11598 Westpark Place Dr., Saint Louis, MO  63146.    The 2 scaffolding systems were not properly supported as follows:    a.)  The 3 poles of the west pump jack scaffold did not have mud sills.  b.)  The 3 poles of the east pump jack scaffold was resting on 3/4" strand board.  c.)  Each pump jack scaffold lacked a baseplate on 1 pole.

29 CFR 1926.452(j)(2):     Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    Location:  At or near 11576-11598 Westpark Place Dr., Saint Louis, MO  63146.    The employer neither braced nor secured the base of the 3 poles on the west pump jack scaffold and 2 poles on the east pump jack scaffolds to ensure that the system was secure and stable.

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    Location:  At or near 11576-11598 Westpark Place Dr., Saint Louis, MO  63146.    The employer did not ensure that workers were protected from fall hazards when performing siding activities using the 2 pump jack scaffolds that had a fall distance greater than 10 ft.  The platform to ground distance ranged from approximately 13'1" to 33'.

29 CFR 1926.1053(b)(5)(i):     Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):    Location:  At or near 11598 Westpark Place Dr., Saint Louis, MO  63146.    The ladder used to access the west pump jack scaffold was not set up at the proper angle.
