On or about 10/9/12, at the Cumming job site, the employer did not ensure scaffold boards did not extend more than 12" over the scaffold supports and instead permitted the boards to extend up to more than 20". In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. On or about 10/9/2012, at the Cumming job site, employees working from a scaffold at levels of 10'8" were not provided a ladder for access and instead climbed the frame of the scaffold to reach working levels. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. On or about 10/9/2012, at the Cumming job site, scaffolds used by employees were not inspected by a competent person in scaffold erection, prior to permitting employees to use the scaffolds. Deficiencies on the scacffold included: no ladder to working levels, no guardrails provided above 10' and improper decking. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. On or about 10/9/2012, at the Cumming job site, an employee was exposed to 10'8" fall hazards from scaffolds that were not provided with guardrails for employee use. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. HBJ Masonry Contracting Company, Inc. was previously cited for a violation of this OSHA standard or it's equivalent standard 29 CFR 1926.451(g)(4)(i) contained in inspection #312775463, citation 1, item 2, issued on 1/26/2010 and became a final order on 2/16/2010.
