29 CFR 1904.29(b)(6):  The employer did not enter privacy case in the space normally used for the employee's name on the OSHA 300 form or equivalent, for an employee who received an injury or illness specified in paragraph (b)(7) of this standard, as a privacy case:        a)  EMStar - The employer did not enter privacy case, and entered the name, for an employee, who received a needlestick injury, on 7/9/12, as specified in 29 CFR 1904.29(b)(7)(v).   The entry included the employee's  name.  Observed on or about 7/27/12.       NOTE:  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(f)(1):  The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):    a)  EMStar - Employees are required to wear N95 respirators, when they enter the hosptial room, or transport patients on respiratory precautions.  The employer provided and required employees to wear Moldex N95 respriators, and did not provide fit-testing, exposing employees to infectious respiratory illnesses.  Observed on or about 7/27/12.      NOTE:  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1030(g)(2)(iv): Annual training for employees with occupational exposure was not provided within one year of their previous training:      a)  EMStar - Employees transport patients via ambulance, and are exposed to blood and body fluids.  The employer did not provide annual training within one year of their previous training.  Observed on or about 7/27/12. 29 CFR 1910.1200(h)(1):  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a)  EMStar - Employees use hazardous chemicals when they disinfect the ambulance, administer oxygen, and perform other operations.  The employer did not ensure that employees were provided with information and training on hazardous chemicals in their work area at the time of their initial assignment.
