29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows. On or about 10/9/2012, at the Cumming job site, two employees were anchored to a single FallTech anchorage device intended for one employee, leaving employees exposed to fall hazards of 12'8". In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level. On or about 10/9/2012, at the Cumming job site, employees using personal fall arrest systems were exposed to fall hazards of 12'8" to lower levels as the rope grab employees were using were attached to the end of the 50' rope used in the fall protection system and did not provide fall protection to lower levels. Abuck Building Systems, Inc. was previously cited for this deficiency in inspection #315742940, citation 1, item 1, issued on 6/20/2012. The citation became a final order on 7/12/2012. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
