29 CFR 1910.269(d)(6)(iv): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Generator - The cover of the generator was removed, exposing the energized heater elements which were not de-energized and locked out, thereby exposing employees to contact with the 480 volt AC energized heater elements, on or about October 5, 2012.
