29 CFR 1910.147(c)(4)(ii):   The procedure(s) shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, specific procedural steps for the shutting down of equipment and placement of lockout devices:    Boiler - The written lockout procedure for the "Boiler Combustion Air Fan" incorrectly identified the main breaker @ boiler control panel as the energy source to be tagged and locked out.  The main breaker at the boiler control panel had been removed on 3/5/12 and lockout needed to be accomplished in the small boiler room.
