29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Quality Roofing, Findlay, Ohio:  On or about October 9, 2012, at the address of 1717 Medical Blvd., Findlay, Ohio, the employer did not ensure that each employee engaged in roofing activities on a commercial structure with a steep sloped roof were protected from falls.  No form of fall protection was being utilized to protect employees from falling approximately 10.6 feet to the paved ground below, which could result in permanent disability or death.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Quality Roofing, Findlay, Ohio:  On or about October 9, 2012, at the address of 1717 Medical Blvd., Findlay, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems.  Employees were exposed to falls of approximately 10.6 feet to the ground below, which could result in permanent disability or death.
