29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    The company did not establish a lockout/tagout program to protect employees involved with servicing and maintenance of machines and equipment from the unexpected energizing, start up or release of stored energy.  Energy sources included, but are not limited to, electrical, pneumatic, hydraulic, etc.  The company maintains and services equipment such as, but not limited to, drill presses, hydraulic presses, and cut off machines

The DEKA drill press 4524, located on the main shop floor did not have a guard protecting the pinch point created by the vertically moving work table and rotating chuck and drill bit.    b. The DEKA drill press 4526, located on the main shop floor did not have a guard protecting the pinch point created by the vertically moving work table and rotating chuck and drill bit.    c. The DEKA drill press 4528, located on the main shop floor did not have a guard protecting the pinch point created by the vertically moving work table and rotating chuck and drill bit.     d. The DEKA drill press 4529, located on the main shop floor did not have a guard protecting the pinch point created by the vertically moving work table and rotating chuck and drill bit.
