1910.178(l)(4)(ii)(A): Refresher training in relevant topics was not provided to operator(s) when the operator was observed operating the vehicle in an unsafe manner as required by 29 CFR 1926.602(d): On or about October 9, 2012 an employee operating a Terex forklift truck lifted an employee of another company on a pallet approximately 20 feet in the air. The employer failed to conduct refresher training for employees involved in the safe operation of powered industrial trucks.
