29 CFR 1926.1053(b)(21):     Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:    The employer allows employees to be exposed to hazardous conditions created by improperly using an aluminum extension ladder.  The most recent occurrence was at  3455 DePaul Lane, Bridgeton, MO, 63044, where two workers were observed climbing up an aluminum extension ladder, carrying and holding multiple foam cushions with one hand, approximately 15 feet above the next lower level.  The workers were exposed to a fall hazard.

29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    Employees were using nail guns to nail shingles on the roof without safety glasses at the job site located at 3455 DePaul Lane, Bridgeton, MO, 63044.    Juan Garcia Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 3158967290, citation 1, item 2, issued on 11/01/2011, and was affirmed as a final order on 01/09/2012, with respect to a workplace located at 331 Bellevue Street, Cape Girardeau, MO  63701.

29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     The employer allows employees to be exposed to a fall hazard created by performing work activity from the roof of the residential apartment structure without properly installing fall protection systems.  The most recent occurrence was at 3455 DePaul Lane, Bridgeton, MO, 63044, where workers were observed on top of the 6:12 pitch roof, approximately 26 feet above ground, wearing safety harnesses with no lanyards attached to an anchor point.    Juan Garcia Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 3158967290, citation 1 Item 4a, issued on 11/01/2011, and was affirmed as a final order on 01/09/2012, with respect to a workplace located at 331 Bellevue Street, Cape Girardeau, MO  63701.
