29 CFR 1910.180(h)(3)(xiv): Before traveling the crane with a load, at Well 10-H located at Briggs Ranch, Catarina, TX 78836, the crane operator who was designated to determine and control safety for the crane was not trained to anticipate the potential safety impact resulting from a third party's failure to properly attach the pin to the legs of the injector head.

29 CFR 1910.180(h)(4)(ii): Employees were permitted to stand or pass under a load on the hook. At Well 10-H, employees walked and worked underneath a load on a hook suspended by a Liebherr mobile crane. An employee sustained a fatal injury when struck by a lose leg of an injector head suspended approximately 60 feet above ground.
