29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Employee was injured on POTDEVIN gluer rollers when the machine was operating with guards off for cleaning 8/30/2012. b) Employee was injured on the gluer rollers while cleaning POTDEVIN gluer that was not turned off 9/13/2012. c) On the POTDEVIN gluer, the 56" wide pan had approximately a 1 3/4" gap between pan edge and roller. d) On the POTDEVIN gluers, conveyors had accessible ingoing nip points on the take away belts. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED
