No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(6):  Unsecured ladder(s) were used on surfaces that were not stable and level:    a)  Employees were using an extension ladder which was set up on a surface that was not level and was not secured.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a)  Employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 23 feet above the ground.     No abatement certification or documentation is required for this item.    Vision Team Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 315508507, citation 1, item 1, issued on April 18, 2011 and became final order on or about May 20, 2011 with respect to a workplace located at 1762-1764 Pavilion Way, Park Ridge, IL 60068.
