29 CFR 1910.1048(j)(3): The employer did not ensure that spills were cleaned promptly by employees trained in proper methods for cleanup and decontamination: On or about September 28, 2012, at the Histopathology Lab, employees (Histology Technicians) required to work in the lab were exposed to a formaldehyde and/or ammonia release in their work area during a Formalin spill and neutralization incident. The employer did not ensure that spills were cleaned promptly by employees trained in proper methods for cleanup and decontamination.
