29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a) On or about Sept. 18, 2012 Eaton Leonard Bender No 5 was not guarded adequately to prevent parts of the operator's body from entering the point of operation during the hazardous motion while a tube was being bent.  An operator suffered a partial finger tip amputation when his finger was pinched by the machine clamp.     b) On or about Oct. 11, 2012 Eaton Leonard Bender No. 4 was not guarded adequately to prevent parts of the operator's body from entering the point of operation during the hazardous motion while a tube was being bent.     c) On or about Oct. 11, 2012 Eaton Leonard Bender No. 14 was not guarded adequately to prevent parts of the operator's body from entering the point of operation during the hazardous motion while a tube was being bent.     Abatement Certification and Abatement Documentation Required.
