29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Jobsite located at 2225 S. Cooper St., Wichita, KS 67207; Employees engaged in residential roofing work on a 9 in 12 pitch roof were exposed to fall hazards of up to approximately 11 feet from the eave to the ground.  Fall protection available at the jobsite was not used.       Artemio Favela was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 19126.501(b)(13), which was contained in OSHA inspection number 314025669, citation number 01, item number 04 and was affirmed as a final order on August 18, 2010, with respect to a workplace located at 3007 Inverness Dr., Hutchinson, KS  67502.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Jobsite located at 2225 S. Cooper St., Wichita, KS 67207; employees were exposed to fall hazards of up to 11 feet from the ladder to the ground below, in that the siderails of the portable extension ladders used to access the roof did not extend at least three feet above the landing surface of the roof:    a) ladder on north side of roof, extended approximately 6 inches.    b) ladder on south side of roof, extended approximately 18 inches.        Artemio Favela was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 314025669, citation number 01, item number 06, and was affirmed as a final order on August 18, 2010, with respect to a workplace located at 3007 Inverness Dr., Hutchinson, KS  67502.

Cooper St., Wichita, KS 67207; Employees performing roofing work were exposed to fall hazards in that employees were not trained to recognize and correct hazards associated with ladder usage.    Artemio Favela was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1060(a), which was contained in OSHA inspection number 681118, citation number 01, item number 07 and was affirmed as a final order on August 18, 2010, with respect to a workplace located at 3007 Inverness Dr., Hutchinson, KS 67502.
