29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of American National Standards Institute, B7.1-1970, Safety Code for the Use, Care and Protection of Abrasive Wheels, and paragraph (d) of this section:    a) On or about October 11, 2012, at 8610 Martin Luther King Jr. Blvd.in Houston, TX; a Ryobi portable grinder, serial number AB082300264, did not have the safety guard installed, exposing employees to struck by hazards.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:    a) On or about October 11, 2012, at 68610 Martin Luther King Jr. Blvd. Houston, TX; the laddder side rails extended less than 24 inches above the upper landing surface, exposing employees to a 15-foot fall hazard.
