29 CFR 1910.95(c)(1):   The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    (a) On or about 10/11/12  - The Saw Operator on the automatic sealed air line was exposed to continuous noise at 149.4% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of  92.9 dBA.  The sampling was performed for 437 minutes during one shift.  Zero exposure was assumed for the unsampled time of 43 minutes.    (b) On or about 10/11/12 - The Saw Operator on the manual sealed air line was exposed to continuous noise at 178.9% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of 94.2 dBA.   The sampling was performed for 438 minutes during one shift.  Zero exposure was assumed for the unsampled time of 42 minutes.     (c) On or about 10/11/12 - The Saw Helper on the automatic sealed air line was exposed to continuous noise at 107.1% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of 90.5 dBA.  The sampling was performed for 435 minutes during one shift.  Zero exposure was assumed for the unsampled time of 45 minutes.    (d) On or about 10/11/12 - The Regrind operator position was exposed to continuous noise at 108.7% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of 90.6 dBA.  The sampling was performed for 382 minutes during one shift.  Zero exposure was assumed for the unsampled time of 98 minutes.
