29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a) At the site, on October 10, 2012, the employer did not ensure an employee working on a steep pitch roof at a height of nineteen feet from the ground level was sufficiently protected from falling, exposing the employee to the hazards associated with death, paralysis, broken bones, concussions, lacerations, and bruises. ABATEMENT CERTIFICATION IS NOT REQUIRED.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: a) At the job site, on or about and at times prior to October 10, 2012, the employer did not provide for at least one handrail on a stairway used by employees to access the second floor of a commercial construction building that was being framed, exposing employees to the hazards associated with bruises and minor lacerations. ABATEMENT CERTIFICATION IS REQUIRED
