On or about 10/09/2012, 5109 Highway 78, Stone Mountain, GA:   a.)  Employees were exposed to a struck-by hazard from material stacked on the H-frame scaffold, the working level was not fully planked.    b.)  Where the scaffold planks were abutted to create a longer platform, a single plank was used and it was resting on another plank, not an upright.

On or about 10/09/2012, 5109 Highway 78, Stone Mountain, GA: Employees working on an H-frame scaffold were not trained to recognize the hazards associated with scaffolding erection including.  Deficiencies observed include but are not limited to the following, scaffold posts and legs did not rest on base plates or mud sills; working level was not fully planked; planking used to make the working platform longer, did not rest on the supports.
