On or about October 9, 2012, the wooden, 2x10 planks, used to extend the platform of the ladder jack scaffold, were bridged to extend the platforms from one ladder jack scaffold to the other. Condition exposed workers to a fall hazard of 8 feet due to shifting, uplift and/or displacement of scaffold, planks and/or members.

29 CFR 1926.452(k)(4): Ladders used to support ladder jacks were not placed, fastened, or equipped with devices to prevent slipping: a) worksite, 1011 West 38th Street in Erie, PA. On or about October 9, 2012, the ladders used to support the ladder jacks and platforms were not secured to prevent them from slipping and had the anti-slip feet turned up while standing on plastic tarps on the ground. Condition exposed workers to a fall of 8 feet due to potential displacement of the ladders.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: a) worksite, 1011 West 38th Street in Erie, PA. On or about October 9, 2012, employees were exposed to an electrical hazard from the electrical cord of the hand-held DeWalt circular saw that was frayed. The electrical cord had been cut and the inner wires were exposed.
