29 CFR 1910.212(a)(1):     Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by:    Instance a:  On September 9, 2012, at the Quick Strut Assembly Department, the employer did not protect the operator and other employees at the point of operation and in the area, including but not limited to Modular Quick Strut machine #5 from struck by hazards such as those created by the compressed strut spring rotating upward out of the machine die.  This condition exposed employees to the hazards of being struck by strut springs being released upward from the machine finger die while under pressure.    Instance b:  On September 13, 2012, at the Quick Strut Assembly Department, the employer did not protect the operator and other employees at the point of operation and in the area, including but not limited to, Modular Quick Strut machine #6 from struck by hazards such as those created by the compressed strut spring rotating upward out of the machine die.  This condition exposed employees to the hazards of being struck by strut springs being released upward from the machine finger die while under pressure.

29 CFR 1910.304(f)(1)(iv):     Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:    On or about October 11, 2012, at the South exterior wall in Quick Strut Assembly Department, the employer did not ensure that the electrical circuit breaker boxes was readily accessible and/or unobstructed to each employee or authorized building management personnel. Electrical circuit breaker boxes including, but not limited to Panel 2-A Circuit Breaker box, Panel 2-B Circuit Breaker Box, et cetera, were obstructed by, but not limited to the following items, boxes with dispensing napkins, snack vending machines, trash receptacles, open cabinets with microwaves, metal shelving. This condition created delay in shutting off the power in an emergency, and  trip and fall hazards in a fire.
