29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      a)	Old Shop, north room, on the Monarch lathe located in the east section, Number 26837, employees were exposed to being caught in or struck by the lathe chuck that was not guarded by a guard or shield.  The lathes chuck has protruding metal jaws for clamping materials.    b)	Old Shop, north room, on the blue Monarch lathe located in the east section, employees were exposed to being caught in or struck by the lathe chuck that was not guarded by a guard or shield.  The lathes chuck has protruding metal jaws for clamping materials.    c)	Old Shop, north room, on the Doria lathe located in the east section, employees were exposed to being caught in or struck by the lathe chuck that was not guarded by a guard or shield.  The lathes chuck has protruding metal jaws for clamping materials.    d)	Old Shop, south wall, on the Bullard vertical turret lathe, the guards were not in place on a 27 inch section of the barrier.    e)	New Shop, west wall, on the Betts vertical boring machine, the machine was being operated with the fabricated guards that install along the rotating base that hold the part in place, were sitting beside the machine, not in use for guarding.

29 CFR 1910.215(b)(9):     The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a)	New Shop, on the Dayton ? HP pedestal grinder, the right side did not have a tongue guard to adjust and the left side was adjusted at a distance of ? inch.    b)	Old Shop, on the Steel City Tool Works, 8 inch pedestal grinder, the tongue guards were missing from each side.  The distance between the grinding wheel and the frame on the left side was ? inch and the right side was 1 ? inches.
