(a) North Interior Corner of Building - On or about October 12, 2012 employees were required to use employee purchased personal fall arrest systems, including body harnesses and lanyards. Body harnesses and lanyards were not provided by the employer at no cost to employees.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: (a) Construction Site - On or about October 12, 2012 through October 15, 2012 employees were exposed to a fall hazard up to 75 feet while wearing fall arrest lanyards showing wear, damage, and other deterioration. The lanyards were not removed from service.
