29 CFR 1926.20(b)(2):     The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    a) Workers were engaged in commercial roofing operations renovating the upper level of an existing structure. (Strip Mall) Located along the upper front and rear area of the roof workers were exposed to a 12.0 foot fall hazard in that regular and frequent inspections of the jobsite were not properly conducted.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.      a) Workers were engaged in commercial construction operations renovating anexisting low sloped roof of an existing strip mall. Located along the front and rear area, workers were exposed to a potential 12.0 foot fall hazard in that a proper fall protection systems was not utilized.
