29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a)  On or about 10/10/12 - Procedures were not developed for employees performing service and maintenance activities on equipment such as the Aida (#218) and Seyi 440 (#220) press.    (b)  On or about 10/10/12 - The Stamtec S2-400 press was not locked out while an employee was unjamming a misfeed a climbed on top of the machinery.    (c) On or about 10/10/12 - The operator of press number #14 was setting up the machinery and did not lock out the machinery.    (d) On or about 10/10/12 - The operator and helper doing set up on press #17 did not lock out the machinery.    The Choice Fabricators was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 311752067, citation number 1, item number 1a and was affirmed as a final order on 06/22/2009, with respect to a workplace located at 3155 Steele Station Road, Rainbow City, AL  35906.
